Housing Support Services
PDS partners with Hennepin County to provide Housing Support Services for individuals experiencing long-term homelessness.
We offer two different Housing Options:
Housing With Services – Independent (HWS-I)
Long-Term Homelessness (LTH)
Our Housing Case Managers help people with disabilities live independently in the community.
Eligibility is determined by the County Coordinated Entry Program.
Housing Support may cover:
Rent and utilities
Food and housekeeping
Bed, bedding, and linens
Laundry and cleaning supplies
Clothing storage
Additional services may include:
Help keeping your housing
Support with daily living and social skills
Job support
Health-related services
To get started, contact the PDS Housing Coordinator or Hennepin County’s Coordinated Entry to see if you’re eligible for the program.
Family Navigation (Family Tenancy Support Services)
Provider: PDS
Program Overview
Family Navigation helps families transition from homelessness into permanent supportive housing. Referred through Hennepin County’s Coordinated Entry System (CES), the program supports families with the highest needs.
Families are matched to one of five housing options:
Minnehaha Townhomes
Family Expansion
Hook & Ladder
Stable Homes
Emergency Housing Vouchers
Who We Serve
This program is for families who:
Have at least one minor child
Are staying in emergency shelter, domestic violence shelter, or unsheltered in Hennepin County
Have at least one adult with a disabling condition
Have been referred by CES for permanent supportive housing
Services Provided
Phase I: Intake & Assessment (1–3 months)
Weekly in-person meetings
Help gathering documents for housing
Background check assistance
Support with payment plans for past landlords
Goal setting for housing, work, health, and family
Help applying for expungement, if needed
Phase II: Transitional Support (2–6 months)
Weekly contact and 2–3 in-person visits per month
Education on tenant rights and shared housing
Lease review and negotiation support
Move-in cost and furnishing assistance
Help with utility setup and address changes
Create a housing stability plan
Phase III: Long-Term Support (6+ months)
Monthly in-person support
Referrals for therapy, mental health, and family services
Help with housing paperwork and recertifications
Ongoing support to stay housed and manage crises
PDS Housing Stabilization Services (HSS)
Housing Stabilization Services (HSS) is a Medical Assistance (MA) benefit to help Minnesotans with disabilities, mental illness, substance use disorders, or seniors find and keep housing in the community.
What PDS HSS Offers
HSS provides support through three main service phases:
1. Housing Consultation (for people without a waiver or care coordinator)
Helps create a Housing Focused Person-Centered Plan and choose a service provider
Includes:
Help gathering required documents
Planning based on housing needs and goals
Provider selection and coordination
Info on rights, appeals, and non-housing supports
2. Housing Transition
Helps you find and move into housing
Includes:
Housing search and applications
Budgeting and lease education
Help with deposits, moving costs, and landlord communication
Support with benefits, applications, and accommodations
3. Housing Sustaining
Helps you stay in your home and avoid eviction
Includes:
Ongoing housing support planning
Help with recertification, landlord issues, or rent problems
Lease education, safety planning, and coaching
Support with income, benefits, and community connections
Who’s Eligible?
You may qualify if you meet all of the following:
18 or older
Enrolled in Minnesota Medical Assistance (MA)
Have a disability or disabling condition, such as:
Mental illness, substance use disorder, developmental disability
Long-term illness or injury
Learning disability (per DHS)
Need help with mobility, communication, decision-making, or behavior
Have housing instability, such as:
Homelessness or risk of eviction
Living in temporary housing
At risk of entering an institution or leaving one soon
What Documents Do You Need?
If you have a CSSP (Coordinated Services and Support Plan):
You’re all set! No additional paperwork required.
If you don’t have a waiver/CSSP, you’ll need:
Proof of MA: PMI number or MA card
Assessment: MnCHOICES, PSN, or Coordinated Entry receipt
Proof of Disability: PSN, Medical Opinion, or SSI/SSDI proof
