Since 1984, we’ve helped individuals—particularly those exiting the criminal justice system—rebuild their lives, find stable housing, and break the cycle of poverty, crime, and homelessness. PDS provides a safe space for growth, recovery, and long-term success for diverse individuals.

    Permanent housing + case management for individuals experiencing long-term homelessness.

  • Housing With Services – Independent (HWS-I)
    Scattered-site housing with flexible support services tailored to each person’s needs.

  • Health Support Services
    Connections to chemical, mental, and physical health care resources.

  • County Economic Assistance
    Guidance to access cash, food, and medical benefits.

  • Social Security & Disability Waiver Support
    Help applying for SSI/SSDI and disability-related services.

  • Landlord Communication & Tenant Advocacy
    Support navigating landlord relations, leases, and tenant rights.

  • Community Resource Referrals
    Links to jobs, education, food shelves, transportation, and more.

Who’s Eligible?

You may qualify if you:

  • Are low-income

  • Have a disability or are an older adult

  • Meet program eligibility through Hennepin County Coordinated Entry

  • Reside in an approved housing location with a certified provider
