PDS Wraparound Services

At PDS, we’re committed to helping individuals and families overcome barriers such as unemployment, housing instability, disability services, and legal or immigration challenges. Our services are culturally responsive, client-centered, and designed to build long-term stability and self-sufficiency.

Employment Services

Our Employment Counselor provides one-on-one support to help clients find and maintain meaningful employment:

  • Job Search & Preparation – Personalized support with resumes, cover letters, and interview skills

  • Career Planning – Explore careers that match your interests and strengths, with ongoing coaching

  • Direct Job Placement – Access to job fairs, employer connections, and placement follow-up

  • Ongoing Support – Help individuals earn trade certificates like CDL (Commercial Driver’s License), CNA, Certified Peer Specialist, and IT certifications

Additional Support Services

Provided by the PDS Social Services Coordinator:

  • Housing Stability – Help with energy assistance, avoiding eviction, and connecting to rent assistance programs to pay past-due rent

  • Food Access – Support for addressing food insecurity

  • Substance Use Treatment Screening & Referrals – Guidance toward recovery services

  • Transportation Assistance – Medical ride coordination and bus pass support

  • Childcare Access – Help finding reliable, affordable care & completing childcare assistance applications

  • Support for Vulnerable Adults & Their Loved Ones – Guardianship & custody forms

  • Communication Support – Free Lifeline phones and discounted internet plans

  • Translation & Application Help – For state and county forms

Public Benefits, Financial Support & MNsure

  • Public Benefits Assistance – Application support for MFIP, SSI, childcare assistance, and more

  • Representative Payee Services – Money management for vulnerable adults to maintain benefits and housing

  • MNsure Navigation – Certified navigators provide free help with:

    • Applying for Medical Assistance or MinnesotaCare

    • Enrolling and renewing coverage

    • Updating your account and reporting life changes

Immigration Support

  • Form N-648 Assistance – For medical disability exemptions from citizenship tests

  • USCIS Form Completion – Green cards, asylum, work authorization

  • Bilingual Citizenship Tutoring – English and U.S. civics

  • Fee Waiver Support – For those who qualify

All immigration services are offered on a sliding-fee scale to ensure affordability.
