Most PDS programs accept self-referrals — you can contact us directly. Agencies, clinicians, and county partners can also refer clients using the PDS referral form.
You do not need a referral for most of our programs. Call (612) 332-9124, email contact@pdsminn.org, or send us a message using the form below. A PDS coordinator will walk you through eligibility.
These programs can only be entered through a county or court referral — please contact the referring agency listed.
