Transitioning families from homelessness into permanent supportive housing.
Referred through Hennepin County's Coordinated Entry System (CES), Family Navigation supports families experiencing homelessness and struggling with housing instability. Families are matched to one of four housing options: Minnehaha Townhomes, Hook & Ladder, Scattered Sites, and Emergency Housing Vouchers.
Funded through Hennepin County — no cost to families in the program.
