Housing Support Services (GRH)

At PDS, we believe in the worth of every person — especially those facing life's toughest challenges. Since 1984, we've helped individuals — particularly those exiting the criminal justice system — rebuild their lives, find stable housing, and break the cycle of poverty, crime, and homelessness.

Cost & funding

Funded through Minnesota Housing Support (GRH) and county programs — no cost to eligible clients beyond the standard resident contribution.

Eligibility is determined through Hennepin County Coordinated Entry; PDS can help you get connected.

What We Offer

  • Long-Term Homeless (LTH) Housing Support — permanent housing + case management
  • Housing With Services – Independent (HWS-I) — scattered-site housing with flexible support
  • Health Support Services — chemical, mental, and physical health care connections
  • County Economic Assistance — guidance to access cash, food, and medical benefits
  • Social Security & Disability Waiver Support — help applying for SSI/SSDI
  • Landlord Communication & Tenant Advocacy
  • Community Resource Referrals — jobs, education, food shelves, transportation, and more

Who's Eligible?

You may qualify if you:

  • Are low-income
  • Have a disability or are an older adult
  • Meet program eligibility through Hennepin County Coordinated Entry
  • Reside in an approved housing location with a certified provider
