Conditional Release Support Services

Culturally responsive, trauma-informed case management with Hennepin County DOCCR.

PDS offers culturally responsive, trauma-informed, and person-centered support for individuals on Conditional Release Supervision through Hennepin County Department of Corrections & Community Rehabilitation (DOCCR). Our goal is to help clients meet court requirements, build stability, and reduce the risk of reoffending while supporting long-term success.

Cost & funding

Funded through Hennepin County DOCCR — no cost to the client.

How to start

Referral only: referrals are accepted directly from Hennepin County DOCCR for court-ordered clients.

Who's Eligible?

Adults (18+) who:

  • Have a pending adult criminal charge in Hennepin County
  • Are court-ordered to Conditional Release Supervision
  • Are assessed as medium or high risk to reoffend

How Our Case Managers Help

  • Find and maintain housing
  • Access employment opportunities
  • Enroll in health insurance
  • Apply for SSI and county benefits
  • Connect with community resources

How Services Are Delivered

  • In the community — Case Managers meet clients at shelters, transitional housing, probation offices, community centers, and other accessible locations
  • Remote services — virtual support options are available

Referral Process

Referrals are accepted directly from the Hennepin County Department of Community Corrections and Rehabilitation. All clients must be court-ordered to Conditional Release Supervision and meet eligibility criteria.
